About West Care and Support Services

Services are designed to maximise independence, reduce loneliness, promote inclusion and provide much needed carer/ family support. 

Support packages are designed and delivered specific to our clients needs and goals - assessed and reviewed regularly by an experienced social worker. 

We currently provide the following services:

• Companionship - in your home or in the community.
• Welfare checks
• Shopping - for you or with you.
• Support to attend appointments.
• Managing correspondence, and support with accessing online services.
• Support to make nutritious meals.
• Support with house work and household tasks.
• Short term support to maximise independence or support following a hospital stay.
• Day trips with transportation and support.
